3 & 4-Year-Old Preschool - Supply List

- 2 face-shot photos of each child
- Backpack that is easy to open
- 1 long sleeve button-down paint shirt with name on it
- 1 labeled change of clothes, including socks & undergarments inside a Ziploc bag
- Copy of birth certificate
- UP-TO-DATE copy of immunizations
- Health appraisal signed by a doctor
- 2 folders with pockets
- 6 Large glue sticks

3-year-olds:
- 1 container of Lysol Wipes
- 1 pack of paper plates
- 1 pack of baby wipes

4-year-olds:
- 3 rolls of paper towel
- 1 container of Lysol Wipes
- 1 pack of baby wipes
- 1 box of macaroni noodles

*Please note:
Children attending Preschool need to have an up-to-date immunization record on file by the 1st day of school and a Health Appraisal (within the last year) signed by your child's doctor.
